Case 74: I Hate Getting Periods, I Want To Die

This is one case of Dysmenorrhea which was not better with acute. Rather didn’t require any acute. The patient was better with only few doses of Constitutional and Intercurrent remedy.

Ms NS, 25 yrs, was a BAMS having her own practice. Came to me on 18/1/08

CHIEF COMPLAINTM
Dysmenorrhea since Menarche, 1992. Pain starts since Day 1 for 3 hours. Cannot move.
Not > by antispasmodic. < When flow starts. < Stress. > Warm water fomentation3.
> Bending down.
CYCLE: 4/28-30. LMP 6/12/07. Flow: Moderate, Bright red, No clots. Non offensive, washes off easily.
MENSES BEFORE: Irritable3 which continues with the pain.
MENSES DURING: Day 1 Weakness++. Weepy3. Wants to die due to pain. Flatulence ++. Appetite reduced.
1/6 months: Diarrhea and vomiting with pain.
Pain > after D1
MENSES AFTER: As if a new person.

PATIENT AS A PERSON
HT: 5’4". WT: 59
SKIN: fair, oily.
HAIR and NAIL: No C/o
PALMS: Hot to touch
PERSPIRATION: Palms³
APPETITE: Normal.
THIRST: Rreduces during menses.
TONGUE: Clean
ACIDITY: < Spicy food.
CRAVING: Sweets³. Warm food.
STOOLS: Constipation, hard stools, < outside food or overeating. Otherwise 1/d satisfactory
SLEEP: Excessive.
DREAMS: Very occasionally of Ghost
THERMAL: Chilly. Cannot tolerate too much cold. < AC flow continuously

DAILY ROUTINE
6.30 am: Wakes up. BF: Chapati/bread butter with tea.
8 am to 4 pm: In Hospital as a RMO
2:30 pm: Lunch: Chapati + Veg
5 pm: Tea+, Biscuit/ Chapati/Dal Rice
6:30 pm: Some snack
7 pm: Clinic
10.30 pm: Dinner
11:30 pm: Sleep

LIFE SITUATION
6/6/1982: Elder of 2 sisters. Younger sis MR since birth. Mo HW. Fa General Practitioner. Fa very short tempered ?due to poor financial Condition. Used to hit Pt. Never forgets. Has rage and anger but controls.
Mo like a friend. Shares everything with her.
1997: Sis died. Pt was in SSC. Must show result to parents and Sis so worked hard. Scored 84%.
1997: Met a boy in a marriage. Introduced by a common friend.
1999: Liked a guy. He was doing BCom then completed MBA while working.
Very close to him. But fights often.
2005: BAMS
2006: Worked in a hospital for 1 year
Jan 2007: Started own practice.
Dec 2007: Worked in a hospital, in the morning and clinic only in the evening. Since the patients were lesser in the morning.

EMOTIONAL STATUS
Irritable³ and short tempered³. Shouts³. Cries with anger. Brooding3. Feels bad if cannot say. Wants to resolve the problem if person is very close.
Affectionate: makes fiends easily. Hugs and kisses
Fear: Fearless in childhood. After Sis’s death fear alone. <Watching horror movies.
