A lovely little 9 yr old walked in, wanly leaning on her equally charming mother's arm, on 14th Oct O5, saying they have recently moved to Mumbai and child has high fever now and has not been well since 2mths and could I please see her immediately? How could I refuse these enchanters? But it meant my case taking had to be short and sweet.
Chief Complaint
- Throat
Since July 05 once a month for 8 days. Pain, Cough³
Vomiting, Phlegm white or yellow, offensive. A/F Mumbai weather, cold water. < am3, < HS3, >warm. Day 2 Fever starts rapidly rising to 103o < 7am, Thirst decreased, Drowsiness, Salivation +. O/E Throat redness ++.
Has been taking repeated antibiotics with no relief. H/O Mumps in Aug 05. - Head
Once a week for ½ an hour. Headache throbbing, Hammering. < afternoon, > Crocin, > Vomit, >sleep.
Patient As Person
HT: 5'7'. WT: 28 kg. Lean. SKIN: Clean and clear - Fair. Pink lips. Tall.
Perspiration: Normal. Non-offensive. Non staining.
Nails: White Spots.
Teeth: Caries +++.
Appetite: poor.
Craving: Butter, cereals, chicken, chocolate, curd, soup, onions, oranges, pastry, rice, salty things, warm drinks3.
Aversion: Sweets. THIRST: Less.
Stool: 1 / day; offensive. URINE: Normal.
Mother's Obstetric History: No labour pains. Caesarian Baby.
Developmental History: Normal.
Sleep: Normal. Talks in sleep. Dreams: Ghost of.
Thermals: C3H2. Covers feet even in fever.
Life Situation And Mental State (Briefly)
Till June 05 they lived in Bangalore, where father was working. He has now been transferred to Mumbai. Both parents trained in the Oberoi School in Delhi. Mother is a Muslim and father a South Indian. They lived with Grandparents in Bangalore. There, mother too used to work, teaching part time in 4 colleges - 2 hrs a week. In Mumbai she has not taken up a job.
The child is very pretty, with fair, delicate skin and brown hair. Tall and friendly. She has adjusted well in the Mumbai school, and has made good friends too. Affectionate3. Sensitive3. Sympathetic3. Careless. Anger3 trifles at. Weeps3 trifles at, > Consolation.
To me this child looked a through and through Pulsatilla
