The After-effects of Palsy

Mr AH, a Muslim Music teacher, 64 y, came to me on 28/5/02
with a attack of Bell’s Palsy 19/5/02. H/o Diabetes. He presented with the
following symptoms:
Left angle of the mouth deviated.
Cannot close Left eye tightly, upper eyelid drooping.
Nostril twitch. But speech almost normal.
There was H/o Bell’s palsy of the Lt side in 1997, for which he took treatment
for three mths + physiotherapy. This time A/F draft of AC on 19/5/02 6 pm.
Admitted hospital for the same. Given Inj Neurobion. With this, he had excessive
sleepiness and urine urge ++.
His other Complaints:
Weakness in both knees with pain in calves and lumbar region with lot of
Lethargy.
< ascending 3 < 7-8pm, < walking 100 steps,
< rising from sitting position. > T Combiflam.
Pt as a person: Ht: 5’6’’, Wt: 88.4 kgs. Has a slight alcoholic
besotted look.
Mole on Rt cheek since three yrs. Warts on the thigh.
teeth Caries ++. Palms and soles warm.
Perspiration: diminished. non staining, non-offensive.
Appetite: less. Thirst: seven glasses/day.
Craving: Hot meal3, Sweets3, takes
extra salt in diet.
Smokes since 40 yrs, and alcoholic since many years, but reduced to 1/6 m months
since five yrs.
Stool: Occ hard stools.
Urine: Every two hrs in the night and cannot control.
Sleep: Good. Salivation-drooling in the sleep.
Dreams: Occ, but unremembered.
Thermals: Likes winter. Wants fan3 always >open air3,
likes AC. but in 2002 <AC, wants covering over chest. Thermal Shift: From CH4
to C2H2 in 2002.
Life Situation and Mental State:
1937: Born in Badra ,district of Kanpur. He was the 5th
of six siblings
1944: Taught music by MU.
1949: Came to Aligarh. Did BA in Urdu. Learnt music with PU.
1954: Fa left home and became a priest. Never came home.
1957: Pt had to perforce start working. Gave music programmes in Lucknow for
five yrs.
1961: Got married.
1964: Came to Mumbai and continued teaching Music. Tried films. Got 16-17 films.
Then re-started teaching music. Now he is well settled.
He is independent. Artistic. Affectionate, but loner. Does not like to talk
much. Suspicious, mistrustful, commanding-took time to have faith in me as a
doctor. Irritable < contradiction. Broods. Fear of fright. Anxious and
worried about children’s future. Obstinate. Crying > consolation.
P/H: Haemorrhoids - operated in 1985.
FH: Mo: Br Asthma. Fa: Koch’s. Sis: Koch’s for 15yrs. Br: renal failure. MU:
oral cancer.
