Leucoderma-Vitiligo

Definition: It is an acquired pigmentary disorder manifested by depigmented patches.

Etiology:
It is believed to be an autoimmune disorder according to neural theory neurochemical mediator destroy melanocyte or inhibit melanin pigmentation.

PRECIPITATING FACTORS
:
1 Family history
2 Injury; boil; abscess; burns - may lead to pigmentary defect.
3 Systemic disorder- diabetes, thyroid dysfunction, pernicious anemia, Addison disease.
4 chemical exposure-contact with rubber and plastic articles eg gloves, condoms, certain germicidal agents may cause depigmentation.
5 Worm infestation-esp in children
6 Improper food-alcohol + non veg;; milk + fruits; salty-spicy foods.
7 Mental stress: anxiety, irritability, quarrelsome nature.

CLINICAL PICTURE:
1-White Macules: mm to cm diameter; round or oval shape with hyperpigmented border.

INVESTIGATION: CBC, RBS, PPBS Thyroid function Test
PROGNOSIS: Poor when strong family history, elder pt, leucoderma on the-non-hairy parts of the body-bony prominence; lips; finger nail and genitalia.

HOMOEOPATHIC TREATMENT
1. Constitutional Remedies.
2. Specific Remedies
3. Miasmatic Remedies.

Constitutional Remedy:
Selected on totality of symptoms but some of the medicines which are frequently used for leucoderma with their guideline related with leucoderma..

1-Silicea - A.F.-anticipation; fright; mortification.
Leucoderma after boil ,abscess injury, vaccination after; due to deficient nutrition. Leucoderma start from finger nails.
2- Merc-Sol: A.F oversensitiveness; fright. vitiligo on the genitalis; white hair in leucoderma spot; h/o chronic digestive problems.
3- Nat-mur: AF grief, mortification, excess use of salt. vitiligo on face and nose, h/o chronic rhinitis; thyroid dysfunction.
4-Nitric-acid AF grief, long lasting anxiety, vitiligo at muco-cutaneous junction, h/o chronic gastric c/o and anemia with bleeding tendency.
5- Kali-carb AF disturbances at work or from family problems. vitiligo-deadly white milky skin without shining, h/o chronic gastric c/o; h/o asthma.
6- Mang-acet AF dissatisfaction, h/o domination by others, vitiligo begins during menopause, from chemical burns, with chronic disease-anemia, kidney disorder, tuberculosis.
7- Argent-nit AF grief, apprehensive anxiety, fear. vitiligo-brown irregular white patches, with gastric c/o, diabetes.
8- Cina vitiligo in children with h/o worm infestation, gastric c/o.
9- Borax AF.fear. AF Vitiligo-red spots are present between white spot.
10- Drosera AF fear with dependency. vitiligo with asthma or tuberculosis.
11- Apis-mel Vitiligo with milky white, bright shiny skin, patches have a surrounding redness.
12- Luticum congenital vitiligo or vitiligo on the genitalia.

SPECIFIC MEDICINES:
If I am unable to get a constitutional remedy. Then in some cases I use the following specific medicine with good results.
1-Ars-sulf-flav -3X in luke warm water
2-Hydrocotyle-A -Q in luke warm water- 5 drops -15-ml of water

ANTI-MIASMATIC MEDICINES:
Selection of anti-miasmatic medicine according to dominant miasm in the case.
My experience for leucoderma: most common anti-miasmatic medicines are Sulphur, Psorinum and Tuberculinum.
