'Bufo
Mr. s. 45 year, married with 3 children, working for MTNL, came for treatment of his breathlessness of 12 years standing, Breathlessness has become acute after H/o Koch's 10 years ago (for which he took treatment for 2 years)
Chief Complaiint: Dyspnoea on least exertion < ascending. Starts with Dyspnoea, followed by coughing bouts and yellowish easy expectoration, present all throughout the year. < Cold wind, < fast fan, < Extreme heat, < strong smells, < early morning 3-4 am, < curds < cold drinks < banana. < Radish. AMEL by expectoration < hot drinks > lying on Lt Side with hands under the head.
Posture: During breathlessness, sits with both hands by his sides, holding the seat of the chair.
Concomitants: Hot feeling with perspiration over the face, scalp, front of chest, desire for fast fan but feels. Onset sometimes related to hearing bad news, and in closed spaces. Frontal headache < chilled < worrying.
Impression: Took time to answer queries, appeared dumb and dull. Had to be pushed to talk.
Past History:
- Recurrent episodes of fever
- An episode of convulsions after violent anger- takes valparin tablets for it.
- Dysentery alternates with chest infection alternate with breathlessness
Skin - Urticarial eruptions, 7-8 years back, white spots over the body with itching red spots.
Lt Side pulmonary Koch's 10 years back
Previous Treatment- Valparin - 2 years for, tits, Theobid 300 TID
Family History: Father - Diabetes
Personal History:
Appetite: loss of
Thirst: scanty
Craving: salt, milk
Aggravated: juicy fruits
Bowels: regular
Perspiration: On face & front of chest
Sleep: disturbed when worried, lies with eyes half open
Addiction: Alcohol, tobacco
Thermal: prefers winter
Investigation: Old fibrotic scarring seen in both upper zones & Lt mid zone. Left dome of diaphragm is pulled up.
Physical Examination: Rhonchi, lower zone. Emphysematous chest. BP 150/80 mm Hg Mentals: Violent anger < least contradiction,<When not heeded by people. Anger expressed in the form of violent speech, beating up, breaking things. Always feels and looks sad. Constant worrying nature, like to be alone, nervous, fear of making mistakes, fear of failure and loss, wants success in his endeavors since is burdened with family responsibilities. Cannot to lerate frivolous losses by his son. Had a very carefree childhood. Eldest child in the family of 6, was fond of reading and enjoyed science and maths, was obstinate but did not shrug off responsibilities. Though father was a farmer, he always aspired to work in Bombay. He had to struggle initially since he got married before he got the job. Recently, a sort of depression has overtaken him resulting in aversion for going to office. Has lost confidence in himself. Frequently comes to get sickness certificates to avoid going to work. Undecided on what he wants to do but plans a lot of things. Disappointed with his son who has incurred financial loss.

Follow Up After July 1997
Mentally he has become very mild, tries to forger everything & forgets his son's financial loss by saying he is worthless. Sleeps normal. Sits normal with hands relaxed on the desk. Blood pressure under control. Followed up till April' 99 but apart from feverishness sometimes, no other problems.
Recap Of Phatak's Description Of Bufo:
A remedy for depravity, due to bad inheritance. It affects circulation, causing ebullations
of heat, flushing (patient felt feverish often). Spasms in children, after nursing angry or frightened mother. Mentals; Idiocy, silly, feeble-minded. Angry if not understood.
