CASES MATERIA MEDICA GENERAL ARTICLES ABSTRACT MISCELLANEOUS Q & A

Similimum 99- June1999, Mumbai
NATIONAL JOURNAL OF HOMOEOPATHY 1999 Jul / Aug VOL VIII NO 4.
Reported by
Dr Nilofer Shaikh

Medicure foundation organized a 1-day All Maharastra Homoeopaths seminar on 13th June 1999 at St Andrew's Audi-torium Bandra. Response was over-whelming and registration was in excess auditorium capacity; 1300 delegates and heavy downpour of rains made the management of delegates almost a nightmare! Excess delegates had to be seated in adjoining classrooms with simultaneous video & audio projection; made with best intentions which did not materialize as video and sound were unclear.
All this resulted in delayed starting of the seminar.
Presentations :

  1. Dr. Rajan Sankaran - Perceiving remedy in practice
  2. Dr. Parinaz Humranwala - Similimum through M.M
  3. Dr. Praful Vijaykar - Physiognomy of Homoeopathy
  4. Dr. Farokh Master - Paediatric cases
Morning Session
Dr Rajan Sankaran- Perceiving The Remedy In Practice
. He mooted the idea of, TOTALITY, not as a numerical one, but as of ascertaining the central disturbance as signified by the concomitants and PQRS symptoms. with weightage to the mentals. and physical generals.. He explained his thinking process through a case of bronchial asthma.

Case: The patient was very hard working (worked 17 hrs/ day), ambitious; and restless, wanted to do something new and different all the time. Dissatisfied, wants to be on the move and face new challenges of life. "I work as if possessed and I cannot stand monotony. I like to work all the seven days in a week", the patient said. He never took any holiday except for his marriage. But now he was tired. He had a dream "As if he is standing in a place where ceiling is very short, he is stuck and can not stand straight." He desired windows to be open.

Dr. Sankaran explained expression, sensation and subconscious reality of the patient. Here the patient feels suffocated, stuck in a routine and a hectic schedule and the same sensation is reflected in "suffocated feeling in the chest". with desire to keep the windows open. Every person has a different perception of the reality, which he calls as his delusion. Identifying this will help in arriving at the similimum. He was given Bacillinum 1M. The first response was that the patient was less tired and his wheezing had stopped. His sore throat and fever cleared. When asked about the pace of his working he said that he was able to work more. On the second follow-up the patient continued to show improvement, had more energy, was able to work effectively for 12-14 hours without wheezing. Bacillinum 1M.2nd dose helped him recover completely not only out of asthma but he also resigned the stressful job! He started his own business, taking regular holidays and enjoying life. His delusion was gone. This case - with sensation of oppression and need for fresh air. Falls under Tubercular miasm. Dr R Sankaran took this opportunity to explain briefly the various miasms, which he had identified apart from the three basic miasms. Each miasm represented a different vision of the reality.( His book - Substance of Homoeopathy details these miasms)

  1. Acute Miasm - Sudden, acute state
  2. Psoric Miasm - Optimistic state
  3. Ringworm Miasm - will/will not - confused state.
  4. Typhoid Miasm - Sub acute Miasm
  5. Sycotic Miasm - State where body tries to cover it up
  6. Cancer miasm - state of enormous responsibility.
  7. Tubercular Miasm - state of oppression how to get out of it, feeling of suffocation, depression
  8. Leprosy Miasm - feeling isolated, disgusted, thrown out of society
  9. Syphilitic Miasm - suicidal / homicidal feeling.
  10. Malaria Miasm - Feeling persecuted. and Restricted
Classification of remedies under 3 categories on the basis of their sources:-
  1. Plant -always connected with sensitivity e.g. Drosera plant traps the insect and kills. Main feeling of Drosera is a choked up sensation, suffocated feeling e.g. Hitler.
  2. Animal - Jealousy, attractiveness; attack e.g. Lach, Tarent
  3. Mineral - Accent on Relation-ship e.g. Phos. Del being isolated on distant island. Identifying the kingdom a patient belongs to helps narrow the choice of the similimum.
Dr Farokh Master gave his own video presentation of a case of a baby with Chronic. Myeloid Dysphasia (Editor: It would be more effective to give a live presentation than a pre-prepared video, which detracts from the personal touch)
The baby had repeated colds, cough and diarrhoea. On examination Spleen ++, Liver ++,. Ascites ++, Appetite - reduced2. Patient was on repeated blood transfusions.

Mind: Child loves traveling, dog puppies, and soft instrumental music. Very particular about keeping things, choosing her own dress and also. What others should wear. Wants cleanliness. Wants attention > caressed. Av-sweet2 Craving plain sugar2. During pregnancy, the mother, who was a CA got promotion, while others senior to her did not. So they all were very hostile towards her and did not co-operate. There was consequently heavy pressure of work and she had no time to relax. This led to frustration and depression. Marked aversion to raw onions during pregnancy.

The acute totality suggested Silicea and Ars-alb and the constitutional remedy was Carcinosin. The child was much better with Carcinosin 1M Hydatid cyst disappeared on USG. Blast cell - N.
Frequency of blood transfusions came down Dr Farokh Master suggested that in such cases it is advisable to take the help of a senior pediatrician to monitor the case. In this case also the pediatrician gave him good support.
Important features of Carcinosin.

  1. Single remedy for - Mildness mask behind
  2. A/F prolonged unresolved grief.
  3. Single child syndrome
  4. Identity lost: of parents' wishes, no desire of their own.
  5. Forced to be a perfectionist
  6. Lots of expectation of them
Afternoon session started, after a very hygienically packed lunch, efficiently served, with a brief presentation by representatives of Intel about the advantage of computers in medical profession.

Dr Parinaz Humranwala dwelt on 2 skin cases with demonstration of Repertorisation done on the dais with Hompath software. One was a case of Alopecia Totalis. This 30-year-old male patient became completely bald in a few months. His hair flew even if he sat under the fan and hence was compelled to cover his head while having meals or else his hair flew to others plates. Various treatments were of no avail.

The history revealed that his parents, both of whom were teachers, were strict in bringing up their children. He did well in his studies and sports. He took a job, which involved a lot of traveling. Later he got married and changed his job. This job was in a multi-national company and he found that he was very junior and did not get the position he deserved. He had a feeling that he was not wanted and started applying for other jobs.

He had a sensation of something hot passing through him whenever he made presentations which were always applauded by the audience. He was by nature compassionate, friendly and liked spicy food and carbonated drinks. He had a growing ambition and anxiety about the future and dreamt of not doing well in business. Sleep was un-refreshing. The knee-jerk prescription was Carcinosin because of strict upbringing; other remedies considered on Repertorisation were Caust, Cal-carb, Puls, Lyco, Nat-mur, Phos and Ars-alb. It was therefore necessary to refer to the Materia Medica to clinch the remedy.
Important features of case were:-

  1. Anxiety ++ about future anticipatory
  2. Strict upbringing
  3. Ambitious with fear of failure
  4. Sympathetic and compassionate
  5. Dreams of business, not doing too well.
  6. Sensation of heat in the body whenever happy.
A sensation of hot water poured over her is a symptom of Phos. Phos has also a feeling of unwanted and a delusion that he was on a distant island for which Dr Kent gives only this remedy. She pointed out a flaw in the construction of the repertory: the symptoms as given by the provers is often broken up while framing the rubrics in the repertories and thus the rubrics may give a distorted picture and not the exact feeling or sensation experienced by the prover. In Allen, under Phos the symptom was that 'Delirious fantasies, while slumbering and waking, as if she were on a distant island; had a great deal of business, was a noble lady, etc'

The patient was prescribed Phos 1M. After a few days suppressed eczema???Appeared on hair roots were seen. When the patient showed no further improvement, Phos 10M was prescribed. On improvement not appearing on places injected with steroids a dose of Syphilinum 1M was given whereafter his hair started growing. At the next follow-up his eczema also improved and hair growth was complete. The Before and After photographs clearly brought out the dramatic improvement.

Features Of The Phos Baby Baby - skin soft, thin, porcelain like > bundled, warmth, massage, fond mother's milk and not willing to take in top feeds. They have low birth weight, and are often born with subconjunctival haemorrhage. She compared Phos with a bottle of champagne, very high spirited and highly enthusiastic. They get intoxicated at parties very easily.

Case 2 A 16-yr. old girl with chronic eczema on her entire body with scabs falling down. There were deep cracks running over the abdomen and on the back She was not able to stand erect as, if she stood erect there was bleeding from the cracks in her abdomen. She had to give up studies because of the eczema and falling of scabs wherever she sat. She was deeply embarrassed and felt ashamed. She could not however give any history for her ailment or other details. She was very reserved, wouldn't talk at all and sat in a corner. Laziness, wants to sit at one place for long time without doing anything. Very shy, timid and won't mix up easily. Anxiety with inferiority complex. Hesitated to do any thing. Aversion to studies. Craving - pickle, ice cream; Aversion- tomatoes. She had constipation; her periods were delayed due to steroids taken for her eczema.

Based on the symptoms such as timidity, irresolute, indolent, slow, eruptions and fissures and her desires and aversions and temperature modality of being a chilly patient, two remedies appeared close viz. She differentiated Kali-silicata & Graphitis Kali-silicata- Aversion to work; muscle gets stretched out; wants to lie down whole day. Kali-silicata came low in marks after repertorisation in this case. Dr Parinaz referred to Kent's discussion of it in his Lectures, where he says that it is a useful remedy for children in school when they cannot learn their lessons, have difficult concentration. Impatient and irresolute. Wants to sit still and do nothing; indisposed to conversation.. She decided on Graphite's on the basis of her constitution and lack of actual drive, not happy doing any work etc..

She was given Graph 200 on 25-2-98. On the first follow-up on 4-3-98 there was no appreciable improvement. Her periods were delayed and it was therefore repeated in 10M potency. Her menses came thereafter. Her skin condition became better gradually. photos of the patient before consultation and after treatment showed clearing of the eruptions with only a few marks remaining. She even appeared for her 10th board exam. This dramatic case, proved the efficacy of Homoeopathic remedies in curing difficult skin condition.

The fourth and last talk was by Dr Praful Vijaykar who explained Physiognomy Of Homoeopathy.
Tall - symbol of purpose, action and emotion.
Short - Stability. Then he gave number of remedies for both statures. He also showed several slides to demonstrate identification of different characteristics of individuals on the basis of their physiognomy, perhaps drawing from the science of phrenology. For want of time he had to rush through several slides and the delegates could not fully grasp the full import of his approach.
