The Single Mother

A lady, aged 40 years, came on 9/12/96 with complaints of breathlessness and wheezing. There was cough with thick, white expectoration, a bitter taste in the mouth. The mucus was difficult to expectorate.
The problems began after her second pregnancy. Breathlessness was < ascending stairs, < winter and
< lying down. It was ameliorated by pressure over chest, putting pillow below head and exercise. There was an itching sensation in the throat, < night. Other complaints included pain and stiffness in lumbar region, < coughing and bending. Hypertension and diabetes complicated these. FBS was 223-mg %, and PPBS was 247.3-mg%. Complaints related to diabetes were headaches, fatigue, hot flushes and excessive irritability. Headaches had begun 4 years earlier and were of a dull, throbbing character, worse noise and sun. Urinary problems
included scanty urination, drop wise with small granular sediments, which she said was like sand.
She had 2 FTND; and a miscarriage a few weeks after the 1st pregnancy.

Past History
: Measles at age of 11 months, chicken pox at age of 1 year and worm infestation in childhood. Bruised [L] arm and head in an accident in 1989.
Repeated follicular pharyngitis. on and off.

Family Hostory
: Revealed bronchial asthma and 1st degree haemorrhoids in father. Mother had scabies, carcinoma of the breast and renal calculus.

Menstrual History
: MP 7/30-34 LMP 6-11-96

: acne, cold, cough, sore throat, weakness and
heaviness of breasts before menses, and lumbar pain during menses > if she passes stool. Fishy, offensive odour of the menstrual discharge, profuse clots and dark red that stained the clothes brownish. She had a history of vaginal itching due to candidiasis.

Life Story
: The patient came from a middle class family. Her father in the construction business and he was dominating. In 1978, she finished B Sc and joined Jaslok hospital as a nurse. She had known her husband since childhood, but souring of the relationship began after her son was born when she went to her parents for 9 months.
Later her BIL tried to harass her and her son sexually. Things worsened and after several showdowns, she separated from her husband. Now, she works in a diagnostic centre, stays with her parents and brings up her two children. Most of the time she maintains a cheerful front but occasionally gets bouts of depression.

20/1/97 > but infection + Kali-carb 200 [1]

12/2/97 R arm elbow pain on pulling+ otherwise & cold Rhus-tox 200 7PIP =2 +Kali-m 6x2/7
19/3/97 Cough+ with urine incontinence. Caust 200 7P 1P=2
10/4/97 > Overall better exc. Diabetes which has not been treated for 3 mths. Kali-carb 200 3P Placebo pills Now in 1998 she gets no cough, diabetes is getting under control.
26/7/97 Kali-c 1M [1] to maintain and bring down blood sugar; repeated as necessary.
