Case 1: Osteoarthritis
Mrs. LHD Age 56 years.
Pain both Knee jts, since 9 years;
[L] knee jt > [R] Knee jt.
X-ray Findings suggestive of OA changes with reduction in joint spaces.
Was taking Allopathy, wants to stop it as it causes acidity.
~ Resp.System. Dry cough since many years. 2-3 times in a year, lasts 15-20 days. Since 5-6 days cough and chest pain, disturbing sleep. AGG if sits for long time
AGG ascending or descending stair
AGG walking ++
AGG winter++
AGG sour++
AGG cold food and cold drinks.
AGG winter
AGG night, disturbing sleep
AMEL drinking water.

Past History: Jaundice 12 yr. back, Malaria 2 yr. back.
Family History: not contributory.
Physical Generals:
Perspiration - face2, no smell, no stain.
Thermal - chilly, likes covering through out the year.
Bath: warm water all through the year, fan slow even in summer. Wants sweater or shawl in winter.
Appetite - good
Craving - Sour++ salt++
Bowel: constipation motion unsatisfactory since 5-6 yr.
Bladder: frequency more at night 3-4 times.
Sleep: disturbed, due to knee joint pains.
cough recent since 5-6 days, and thoughts of. family tension.
Dreams: gets but does not remember.
Obstetric History - 4 Ftnd: 2 males, 2 females. No h/o AGG. during pregnancy.
Menstrual History: Menopausal for7 yr. No AGG then. H/O regular menses.
No AGG BM or DM.

Family Set-Up: Pt stays with husband, younger son, DIL and 2 grandsons. 2 daughters married and elder son with his family staying separately. Relations are good with everyone. Recently elder son having conflicts with his Sister [pt's D] . This has nothing to do with patient, but she feels disturbed, also disturbing her sleep. Pt. is of calm and quiet nature, gets irritated only if grand son is mischievous and then scolds him.
O/E - Swelling++, Pain +, Stiffness3,
Both Knees - Swelling ++; Crepitus +
Resp System : N.
CVS : N; BP 130/90 mm/ Hg
Wt. - 76 kg Ht. - 5' 2"
Skin and palms - warm.
Varicose veins in popliteal fossae ++

Totality Considered:
Chilly pt.
Bb Pg. 1152 AGG winter
Bb Pg. 1122 AGG sour
Bb Pg. 1120 AGG food cold
SR II pg. 269 Cr sour
SR II Pg. 266 Cr salt

Drugs After Repertorisation [Chilly]
Ars 4/15
Carbo-veg 4/14
Calc-carb 5/11
Hep-sulph 4/15
Phos 4/11
Rhus-tox 4/11
Sep 4/11

Patient's fat and flabby built made me go for Calc-carb. From her life situation, I got the impression that she is sensitive and affectionate. She feels the pains of her children i.e. elder son and D which disturbed her sleep but she did not take any active step in dealing with them.This showed her indolence which further confirmed Calc-carb.

Criteria for Follow-up:
1] chest pain
2] cough
3] sleep
4] motion
5] urine freq at night
6] knee jt pain
7] knee jt swelling

Pt was put on Calc-carb 30 TDS on 25/2/97
Report on symptoms 1 2 3 4 5 6 7
7/3/97 > > > S S S S

Since recent complaints were better with constitutional remedy but there was no registration at joint level, 1 dose of Oan 1M was given as intercurrent followed by
Calc-carb 30 TDS X 7 days.

15/3/97 > 3 good > > > 25%

This time all her other complaints were better indicating that constitutional was helping her at all levels But pt was dissatisfied because swelling was same.
Calc-carb 30 TDS continued. 22/3/97 knee jt pain better 50%
Swelling same.
Dissatisfaction continues. Son and DIL report that she can walk much more compared to earlier.

28/3/97 Swelling was same. Pain increased as pt. had been out and had walked much.
Stiffness in the knee jt had increased which was more on getting up and gradually lessened as she walked about.
Rhus-tox 30 TDS * 4 days.

5/4/97 acute > swelling same since then pt. keeps reporting and is still under observation her pain is much better. With Calc-carb 30 TDS > Calc-carb 200 daily HS. Requires Rhus-tox SOS if pains increase due to exertion, but there is no appreciable change in the swelling of her knee jt.
