Shuddering After Urination

Mr R, 22 years, a bachelor and one of my distant relatives came to me on 30/1/96 with C/O pain in abdomen of which he has had frequent episodes in the past. He had a long H/O helminthiasis since childhood [grinding of teeth, pica and small worms in stools]. He had taken a lot of Allopathic treatment in the past. This time he wanted to have a permanent solution to it. His symptoms were -gradually increasing pain since 3 days esp. around umbilicus, with no relation to food or time. Generally was constipated, had offensive flatus and the pain amel. while rising from lying . It was agg. since morning after taking Zinetac tabs. O/E -Abdomen was distended and slightly tender. Liver/Spleen-not palpable. Prescription of Colocynth, Argentum-met and Carbo-veg during the course of the day [after reviewing each time] did no good. The first episode of pain was so severe that later, in the middle of the night I had to battle it out with him for nearly 2 hours. And after having tried not less than half a dozen remedies [Bell, Acon, Nux, Puls, Mag-p all in 200 potency] finally, Cina 200 seemed to give him some relief. The next morning, Cina 200 helped him further and he passed a lot of worms in the stools.
There were a few more episodes of pains but not of such intensity as to require medication.
The symptom relapsed again on 26/6/96 and this time Cina 1M, then Bryonia 200 did no good and the pain was again gradually beginning to peak. It was then that I decided to search for a solution somewhere outside this tried sphere of symptoms [pain abdomen, worms etc.]
Having known him for quite a long time, I knew his background. His mother had committed suicide when he was just a few months old and he was brought up by his paternal uncle. Even as a child he was very violent trying to cause physical harm to others by biting or hitting when angry. A sexual pervert and big-mouthed, he was very bad in studies and had with great difficulty managed to pass Xth and that too compartmentally.
But with this much, I could hardly think of any remedy with confidence. I wanted to know more. So when I asked, "What type of a person are you?" I was pleasantly surprised with his crisp and clear statements which helped me to speedily conclude upon the right drug. To my question his answer was "I like to dominate! I hate it when people look down upon me". He becomes violent when agitated and tries to inflict injury upon others or even gets impulses to kill in that state of excitement [he once stabbed his own sister on her hand with a kitchen knife] Eyes water and body trembles when angry.Does not sustain in any work and is averse to taking up responsibilities.
Absent-minded [+++]
On the physical front he is a warm patient [2-3+] and desires open air. One very interesting and peculiar symptom which he volunteered was that he used to always shudder [kind of shiver] after urination [not the physiological chill which I specifically clarified a number of times, which is seen generally at the beginning of urination.]
Perspiration ++, offensive. Desires - bitter. Aversion - Sour. Pain in the sub-costal region while running.
Analysis: What struck me most was his domineering attitude and his impulses to kill. His absent mindedness [with which he was facing a lot of trouble] and sexual perversions [Lasciviousness] were further adding to his drug picture. That the peculiar symptom of shuddering after urination [Chill, Urination, after KR Pg. 1275] confirmed the choice of the drug and was an added bonus.
So on 27/6/96, evening - Platina 200, 4 doses [BD] saw an end to the tormenting pains, with no further relapses thereafter. NB- The gradually increasing pains, constipation and worms were some more symptoms of Platina in this case]
Now it is nearly 7 months since I last saw him; before writing this article, I asked him if he had noticed any other changes apart from relief from the main complaints. On the mental front, he says, he is the same, his domineering attitude and absent mindedness haven't changed though there haven't been any more instances of violent behaviour. The shuddering after urination also has not gone [it has been there since his childhood]
Conclusion: 1] The ultimate basis of the selection of drug was so different from the presenting complaint [of helminthiasis with pain in abdomen] that it again speaks of the oft repeated mistake of partial case taking in such seemingly simple cases.
2] Another point of interest in this case is to know, if at this age, the mental attitude of the patient can be improved further [his domineering nature, aggresiveness and absent mindedness] with 10M and higher potencies of Platina. With hardly 2 years of practice, I would like the esteemed readers and the senior and experienced practitioners to provide an answer if possible.
