Approach to sexual problems - Mumbai August 96

A panel discussion on Approach to :Common Sexual Problems" was organized by the Santacruz Medical and Social Association in August 1996.

Sexual problems, though personal are important to be solved for human well-being. They range from physical to psychological and in most can be sorted out by a little common sense and understanding. This statement was clarified by a panel discussion.

Dr Vihang Vahia, a psychiatrist opened the session and defined sexuality as a desire for gratification. He explained Freuds theory of psychosexual development from birth to old age (the oral, anal and genital phases).

He described sexual intercourse in a lighter vein as equal to "penis plus vagina plus acrobatics". He explained through cases how psychological understanding of the problem and counselling in the right direction satisfactorily resolved many sexual problems.

Dr Rupin Shah, an Andrologist, explained that in a few cases the physical causes of sexual problems could be remedied by surgical approach. Erectile dysfunction or incomplete erection could be treated by injecting drugs like papaverine directly into the penis. This could be done by patient or the spouse if necessary just before sexual intercourse. Investigative techniques like NTP (nocturnal penile tumescence), Doppler studies and angiograms would be able to detect erectile impotency; arterial blocks,or venous insufficiency, retrograde flow, leakage - faulty valves etc and in most cases could be helped by reconstructive surgery.

Penile prosthesis inserted into the corpora cavernosae could also help the patient to a great extent. The above facts were demonstrated by a slide presentation.

Dr Prakash Kothari,the world renowned sexologist, emphasized that most sexual problems exist in the mind and can be cured without drugs. Reassurance and teaching the right approach through examples, slides, films and models would go a long way in helping to dispel many myths about sexual behavior and attitudes and solve many sexual problems. He cautioned against the use of creams and lotions. Ayurveda has many good remedies provided they are authentic, carefully prepared and dispensed by a knowledgeable Ayurvedic physician. Homoeopathic medicine have also to be given with caution - not as advanced by the "popular newspaper columns" but as constitutional remedies after detailed case history and examination by a well qualified Homoeopath.
