Selection of Similimum would have been easy, provided patients used the same wordings and description as in the Materia Medica. A drug has got so many key note symptoms, but words and description we get from a patient are few and different. Hence the only way is to approach a case from different angles according to the presentation of each case. Leaving aside the theoretical aspects, a few practical points are illustrated below:.
Minimum Symptoms of maximum value is the thumb rule. As per Section 153 Organon : Strange, rare and peculiar symptoms should be given the maximum value. A case of Hansens Disease was given much relief on the symptom - mouth intensely dry without thirst and burning soles - Pulsatilla.
Striking Mental Symptom. A fourteen years old boy, suffering from chronic headache was given much relief with Lyco 1M single dose. He was abusing and insulting his mother in the clinic, baselessly; his father had expired long ago and mother was looking after boy.

Am-m, Hyos, Lyco, Nat m, Plat.
Out of my repeated Clinical experience Lyco was selected. I feel no other drug is having it so strikingly Platina second to Lyco only.
Also see the state of mind. A lady with Chronic migraine was cured with Silica - married yet expressed lot of ambition for future studies although tired. She was timid yet gentle, and expressed a liking for my affection.
At times it may be the Miasmatic block we have eradicate. Many a time while too busy in the clinic, I may look at the black, tiny, horny wart like growths on the neck of the lady nearing her forties (when mostly sycotic Miasm is expressive) and prescribe Acid Nitricum, Thuja or Medo. Can anyone treat pediatric cases without Aviaire, Bacillinum etc? Eruptions between the age of two to five years many times yield to Thuja and for bad effects of Vaccination.
Never well since in a lady, even after insulin therapy, blood sugar was shooting high. She developed all the troubles since Measles - Morbillinum 10M 50M, CM gave excellent relief although cure was not complete.
Using the advantage of Relationship of Medicines is another approach. Sulphur, Calc, Lyco. Or Merc, Hep, Silicaetc.
Never prescribe on pathological symptoms if you can prescribe on the Constitution and temperament. A sweet girl of seventeen came to me with a cystic growth below her left breast. Rapid emaciation and cough accompanied? She was tall, fair, loving and lovable. When asked if I could see the growth, she immediately exposed including part of her left breast. Shamelessness! Phos 30, a few doses completely cured her with a dose of Tuberculinum 1M.
Aetiology is another helping hand. A patient of Migraine, 40 years, is doing well on Ignatia for the last few months, based on the causation that at age of twelve, his house was looted and set on fire by some thugs but the migraine started at the age of 33 only.
Of course use of Repertory based on Mentals, Generals and Particulars is excellent. But when no clear cut symptoms are available use of specifics (???) comes in. Menieres syndrome can be successfully treated with Sodium Salicyclicum 30. Eruptions on the elbow may be treated with Psorinum.
When indicated remedies fail, use of Organ remedies is of importance. Aur mur nat 3X in fibroid uteri; Crataegus Tincture for heart; Aegle marmelos Tincture worked well in a case of chronic dysentery when even the best selected remedy failed. I am not a good user of Biochemic remedies, but so many of my friends do well on that.
Use of Bowel nosodes give excellent results - like Morgan Pure when Sulphur, Petroleum and Psorinum fail. Use any nosode if available with you and try. Trichomonas vaginalis 30 is excellent in leucorrhoea. Herpes 30 is good in mouth ulcers.
Dreams and Positions in Sleep will help us many times. A case of Arthritis was cured based on the dream falling into water. A serious Cardiac case was cured based on the dream - vomiting blood. A male patient was cured with a single dose of Puls 200 for his chronic numbness of lower limbs based on the position in sleep he always had to sleep with his hands crossed over the abdomen (synthesis - 1401).
Location, Sensation and Modality is another approach. A crack behind the ear Graph. Dr. Nash says under Apis, the leading characteristic is to be found in its sensation - burning, stinging pains. Again under Bryonia, As in Puls, so in Bry the leading characteristic lies in its modality - aggravation from motion..
Always latest symptom should be given maximum importance. We must concentrate on the basic complaint. If a cough is the basic complaint in a person of liver troubles, Ammonium-mur will help.
Concomitant Symptom is of much value. A patient with Polyarthralgia improved with Drosera based on the history of Primary complex and was finally cured with Ammon-mur. - he gets an attack of cough before pain starts. Read in Lippes Key notes: Many group of symptoms are accompanied by cough under Ammon mur.
Weather conditions, aggravation of complaints before, during or after menses are very important. Aggravation summer - Bry (diarrhoea). Acne on face before menses, and nasal blockage at night was cured with Mag mur 30.
Flower remedies in this tension era is important. Whenever you hear Ghabrahat try Rock Rose. A patient came for painful and distended abdomen - usual remedies failed. He had an embittered feeling towards all his children - lack of real love. Holly, a single dose on tongue gave instant relief and when continued was cured.
TO PUT IN ONE LINE - MINIMUM SYMPTOMS OF MAXIMUM VALUE is the basic rule.
